Reconozco y entiendo que el Jiu Jitsu y las artes marciales mixtas (MMA) son deportes de contacto que involucran riesgos inherentes, incluyendo, pero no limitado a, lesiones físicas, fracturas, contusiones, torceduras, cortaduras y lesiones musculares. Además, comprendo que tales actividades pueden resultar en lesiones graves o discapacidad. Acepto que 10th Planet Barcelona, sus propietarios, instructores, empleados y cualquier persona asociada con el gimnasio no son responsables de cualquier lesión, daño, pérdida o muerte que pueda ocurrir durante mi participación en las actividades ofrecidas por el gimnasio. Entiendo que es mi responsabilidad asegurarme de que estoy físicamente capacitado y en buena salud para participar en estas actividades. Me comprometo a seguir todas las instrucciones dadas por los instructores de 10th Planet Barcelona y a respetar las reglas y regulaciones establecidas por el gimnasio. I, {first_name}, as a participant in the activities offered by 10th Planet Barcelona, located at Sant Pere Màrtir Street 37, Barcelona, freely and voluntarily agree to take part in the classes and events related to Jiu Jitsu and Mixed Martial Arts (MMA) offered by the gym. I acknowledge and understand that Jiu Jitsu and MMA are contact sports that involve inherent risks, including but not limited to physical injuries, fractures, bruises, sprains, cuts, and muscle injuries. I also understand that such activities may result in serious injury or disability.
I accept that 10th Planet Barcelona, its owners, instructors, employees, and any person associated with the gym are not responsible for any injury, damage, loss, or death that may occur during my participation in the activities offered by the gym. I understand that it is my responsibility to ensure that I am physically fit and in good health to take part in these activities. I agree to follow all instructions given by the instructors at 10th Planet Barcelona and to respect the rules and regulations established by the gym.
I hereby release, discharge, and waive any and all claims, demands, liabilities, legal actions, and rights of action of any kind that may arise against 10th Planet Barcelona, its owners, instructors, employees, and any person associated with the gym, whether known or unknown, present or future, resulting from my participation in the activities offered by the gym.
I declare that I have read this document in its entirety, that I understand its contents, and that I sign it voluntarily and without any coercion.
