El/la usuario/a declara haber sido debidamente informado/a por 10th Planet Jiu Jitsu Barcelona, gestionada por el Club Esportiu de Jiu Jitsu 10th Planet (“la Academia”), de la posibilidad de contratar un Seguro Médico Deportivo anual ofrecido por la Academia, con un coste de veinticinco euros (€25) por año natural. Dicho seguro cubre exclusivamente accidentes y lesiones derivadas de la práctica deportiva dentro de las instalaciones de la Academia o en actividades organizadas por esta, incluyendo atención médica primaria, derivación a centros concertados y pruebas diagnósticas relacionadas con el accidente deportivo, conforme a los términos, condiciones y límites establecidos por la aseguradora correspondiente. El/la usuario/a reconoce que este seguro no constituye ni sustituye un seguro médico privado integral.
El/la usuario/a comprende que la práctica del Brazilian Jiu Jitsu, grappling y disciplinas asociadas implica riesgos inherentes, incluyendo, sin carácter limitativo, lesiones musculares, articulares, contusiones, esguinces, luxaciones u otras lesiones derivadas del contacto físico y la naturaleza propia de la actividad, y participa en ella de forma voluntaria y bajo su propia responsabilidad.
Al aceptar el presente documento, el/la usuario/a elige libremente una de las siguientes opciones:(Selecciona una opción)
ACEPTO contratar el Seguro Médico Deportivo ofrecido por la Academia y autorizo el cargo anual de €75.
RECHAZO el Seguro Médico Deportivo ofrecido por la Academia por disponer de un seguro médico privado propio que cubre la práctica deportiva, asumiendo íntegramente cualquier gasto médico derivado de mi participación en las actividades de la Academia.
En caso de rechazo del seguro ofrecido por la Academia, el/la usuario/a exonera expresamente a 10th Planet Jiu Jitsu Barcelona, al Club Esportiu de Jiu Jitsu 10th Planet, así como a sus representantes legales, instructores y personal, de cualquier responsabilidad económica derivada de accidentes, lesiones o gastos médicos ocurridos durante la práctica deportiva o actividades organizadas por la Academia.
Al firmar el presente documento, declaro que he leído, comprendido y acepto íntegramente su contenido.

The user declares that they have been duly informed by 10th Planet Jiu Jitsu Barcelona, managed by Club Esportiu de Jiu Jitsu 10th Planet (“the Academy”), of the option to contract an annual Sports Medical Insurance offered by the Academy, at a cost of twenty-five euros (€25) per calendar year. This insurance exclusively covers accidents and injuries arising from sports practice within the Academy’s facilities or during activities organized by the Academy, including primary medical care, referral to affiliated medical centers when applicable, and diagnostic tests related to the sports accident, in accordance with the terms, conditions, and limits established by the corresponding insurance provider. The user acknowledges that this insurance does not constitute nor replace comprehensive private medical insurance.
The user understands that the practice of Brazilian Jiu Jitsu, grappling, and related disciplines involves inherent risks, including, without limitation, muscular and joint injuries, contusions, sprains, dislocations, or other injuries resulting from physical contact and the nature of the activity, and participates voluntarily and under their own responsibility.
By accepting this document, the user freely chooses one of the following options:

I ACCEPT to contract the Sports Medical Insurance offered by the Academy and authorize the annual charge of €75.
I DECLINE the Sports Medical Insurance offered by the Academy because I have my own private medical insurance that covers sports practice, assuming full responsibility for any medical expenses arising from my participation in the Academy’s activities.
In the event of declining the insurance offered by the Academy, the user expressly releases 10th Planet Jiu Jitsu Barcelona, Club Esportiu de Jiu Jitsu 10th Planet, as well as its legal representatives, instructors, and staff, from any financial liability arising from accidents, injuries, or medical expenses occurring during sports practice or activities organized by the Academy, except in cases of willful misconduct or gross negligence in accordance with applicable law.
